A person's ability to choose their experiences during a psychedelic trip is limited. A person may find oneself flying through the cosmos, confronting the nightmares that torment them, or some bizarre combination of the two.
Though generally less dramatic, the conclusion usually happens. However, the trip's visceral impacts haven't totally subsided as the consciousness-altering medication starts to wear off. Especially for individuals receiving psychedelic-assisted psychotherapy, this is actually a crucial time. The deep insights and emotions that a person had on their trip may start to fade during these transitional minutes or hours. Sometimes they are unable to put into words what they feel deep down in their bones or hearts. It can be challenging to use a vacation as a catalyst for revolutionary change for some people due to the trip's elusive character. Integration, or the therapeutic process by which people interpret their trip, is often created to aid patients in recalling and interpreting their insights, but as patients grow further away from dosage, their memories aren't always as clear-cut.
This problem was first identified a few years ago by psychiatrist Dr. Prash Puspanathan and scientist Agnieszka Sekula, who formed Enosis Therapeutics, an Australian firm that creates virtual reality settings to support psychedelic treatment. In addition to developing a methodology for psychedelic therapy delivery in virtual reality, they also developed AnchoringVR, a proprietary programme that generates an immersive environment for patients to explore once they start to come down from their trip.
With any commercially available VR headset, a patient begins the AnchoringVR scenario at the end of their psychedelic trip, which can be four to five hours after receiving psilocybin or just after receiving an intravenous infusion of ketamine.
The patient can then use audio and visual components to create their own virtual reality (VR) world. They might decide to sit by the water and virtually interact with a giant rock before labelling it with an emotional burden, such as a particular trauma, and tossing it into the waves. You may pick a star out of the night sky and put it next to an audio clip of a thought or emotion that surfaced while you were away. Then, the patient's North Star is symbolically transformed into that object, which now serves as a symbolic representation of a crucial revelation. The small number of patients who have used AnchoringVR did so for 45 minutes at the conclusion of their psychedelic trip, then rewatched the recordings with their therapist during each integration session.
When a patient is enclosed in a cocoon that is a manifestation of their own thinking, they feel empowered to talk freely and comfortably, according to Puspanathan, the company's lone investor. Puspanathan (who founded Enosis Therapeutics) previously served as CEO of a thriving small-scale bitcoin brokerage.
Enosis has established itself in a surprising market amid the billion-dollar race to find potent psychedelic substances. Though consciousness-altering states can be attained or enhanced with virtual reality (VR), using the technology in conjunction with psychedelic-assisted psychotherapy is untested area. In order to perfect integration, Enosis intends to use the wonder and amazement that VR creates. Although this phase of treatment doesn't hold the public's attention in the same way that the search for new psychedelic drugs or the wonder of a psychedelic trip itself does, doctors believe it's essential to a successful course of therapy. Puspanathan and Sekula are raising significant issues about why that part of the psychedelic experience matters so much and how VR could play a crucial role in enhancing it in their efforts to develop technology that facilitates integration.
How does psychedelic integration work?
It was a "no brainer," according to Dr. Sergio Pérez Rosal, CEO, cofounder, and medical director of the OVID Clinic in Germany, to introduce Enosis to the psychiatric and psychotherapy clinic after he personally tested AnchoringVR. According to Pérez Rosal, the two patients who underwent four sessions of integration utilising AnchoringVR were completely engrossed in the procedure. In order to record their observations, they have interacted with and labelled stars. For maintenance, certain stars were transformed into plants. Some people were burned alive, while others were turned into stones. Additionally, they have developed their own personal symbols that have importance for them, such as drawings of hearts, romantic interests, animals, and birds. This method of interaction is consistent with OVID's strategy for psychedelic-assisted psychotherapy, which integrates pharmaceutical and psychological treatment with a well-integrated psychedelic experience.
"We move the emphasis away from this concept that [the psychedelic drug] is going to effect the transformation," explains Pérez Rosal.
High-quality integration is essential for assisting patients in drawing conclusions from their insights and making new or different decisions in their lives, even though some scientific research suggests that psychedelics may have a potent effect on neurones in the brain, potentially creating new pathways or repairing damaged or dysfunctional ones. Patients are given ketamine at the OVID Clinic, a psychedelic-inducing anaesthetic, to address their conditions. In order to maximise the efficacy of their treatment, patients participate in both integration and pre-treatment sessions.
Prior to joining the clinical advisory board for Enosis, Dr. Margaret Ross, a prominent clinical psychologist at St. Vincent's Hospital in Melbourne, Australia, and the primary principal investigator for the nation's first psilocybin-assisted psychotherapy trial, had known Puspanathan for many years. As a psychologist who cares for patients nearing the end of their lives, she is particularly concerned with making sure that their integration process is as successful as possible. The technology of Enosis, in her opinion, is "very exceptional" in this regard.
Because of its nonverbal and symbolic design, which Ross characterises as "bridging the conceptual and actual without making it too concrete or reducing it all to words," patients may benefit from it. Ross claims that patients who are desperate for psychedelic-assisted therapy to alleviate their severe anxiety or treatment-resistant depression frequently speak to her with the misconception that the drug will automatically result in a transformation. What they fail to realise is how much emotional labour it is to go through with such changes long after the effects of psychedelics have worn off.
Unrelated to Enosis' investigation, researchers are looking into the effectiveness of methylenedioxymethamphetamine (MDMA)-assisted therapy for the treatment of post-traumatic stress disorder (PTSD) in a clinical trial funded by the Multidisciplinary Association for Psychedelic Studies (MAPS). According to Sarah Gael, a psychedelic therapist and one of the trial's researchers, participants have three therapy sessions for integration following each dose. To aid with the integration process, participants can meditate, write in a diary, make art, or engage in movement-based exercises. What's more, it's not always necessary or practical to analyse discoveries with language.
Gael, MAPS's harm reduction officer, claims that after psychedelic-assisted therapy, a patient leaves with a "handful of seeds" that may be fresh perspectives, awareness, alterations in their self-perception, or even mystical spiritual experiences that might aid in healing and personal development. Those seeds will eventually bloom if placed in healthy soil with the proper conditions. In a similar way, Gael claims that integration might foster the ideal atmosphere for change.
